Rates
Dr. Sedlak makes a final recommendation of services needed based on brief consultation with patient. She may recommend a more thorough/specific type of evaluation depending on patient’s needs. Pricing is generally based on a $250/hr clinical rate, though flat rates are offered for most types of evaluations. Specific needs outside these options can be discussed individually with Dr. Sedlak. A good faith estimate is provided before the start of any services in accordance with the No Surprises Act.
Accommodations Evaluation (Ages 16+)
Comprehensive evaluation which meets requirements for accommodations requests on standardized testing (e.g., LSAT, Bar, MCAT, SAT/ACT, etc.). Evaluation includes intake, testing, feedback, and formal report with recommendations, as well as completion of all necessary forms and an appeal letter as needed. Patient is responsible for providing all applicable forms prior to the start of the evaluation to ensure evaluation meets all requirements. It is strongly recommended that a close family member (e.g., parent or spouse/partner) completes a questionnaire to provide additional data as required by most testing committees.

ADHD Evaluation (Adult)
Basic evaluation which includes intake, testing, and feedback (all completed on the same day in person) and formal report with recommendations/resources. Typically used for confirmation of previous diagnosis or to obtain treatment.
While this evaluation screens for depression and anxiety, it does not include testing for learning problems, autism, or an in-depth assessment of mood-related concerns (e.g., bipolar disorder vs. ADHD). A more in-depth assessment of mood/personality can be added by request and costs an additional $500.

Autism Evaluation (Adult)
Comprehensive evaluation to assess for the presence of autism or other developmental factors that may impact learning and behavior; participation by a close family member (e.g., parent or spouse/partner) is strongly recommended. Includes intake, testing, feedback, and formal report with recommendations/resources.

Developmental Evaluation (Child)
Comprehensive evaluation to assess for the presence of autism, ADHD, learning disorders, or other developmental factors that may impact learning and behavior. Collateral information will be gathered from parents and teachers, whenever possible. Includes intake, testing, feedback, and formal report with recommendations/resources.

Concussion Baseline Evaluation (pre-injury)
Screening evaluation to establish baseline cognitive performance prior to participation in sports or other injury-prone activities. Includes intake, testing, feedback, and one-page summary of results.

Neuropsychological Evaluation for Other Medical Conditions
Comprehensive evaluation to assess for cognitive changes secondary to a variety of conditions (e.g., concussion, dementia, MS, other medical factors). Includes intake, testing, feedback, and formal report with recommendations/resources. Participation by a close family member (e.g., parent or spouse/partner) and review of medical records is strongly recommended.

Brief screening evaluation to determine the need for a more thorough evaluation; all aspects are completed in a single in-person appointment. Participation by a close family member (e.g., child or spouse/partner) is strongly recommended. Includes brief intake (clarifying questions based on intake questionnaire), brief testing, feedback immediately after testing, and a one-page summary with results and recommendations/next steps.

Capacity Evaluation
Comprehensive evaluation to assess patient’s capacity for a variety of legal issues (e.g., need for guardianship, financial capacity, medical capacity, etc.). Includes intake, interview with family member/support person, testing, feedback, and a formal report with recommendations/resources. Can be used to support a petition for guardianship, power of attorney, or other legal issue (e.g., application for disability benefits).
