Triad Neuropsychology and Forensic Services is a private neuropsychological practice based in Fuquay-Varina, NC, which is centrally located 20 minutes south of Raleigh.  Dr. Sedlak is also willing and able to travel around the state (primarily to major cities) to provide services in convenient locations to those in need.  A non-refundable convenience fee of $250 is assessed for any office within 100 miles of the home office to cover the cost of the office space.  For offices outside of this area, additional fees may be charged to compensate for provider’s time/expense, which can be discussed with the provider during initial consultation.

Dr. Sedlak’s primary office is located on the second floor in Suite 200.  There is ample parking space and the office is accessible via stairs or elevator.
